Breast Implant Revision

Breast implants are not designed to last forever, just like any other medical device.  Most manufacturers suggest that your implants have a 10 to 15 year life span and will need to be exchanged for safety and/or aesthetic reasons. Some implants could have a complication such as a rupture (silicone-gel implants) or a deflation (saline implants).  Some women want a breast implant exchange because they do not like their current implant size and want something smaller or larger. While other women develop a significant capsular contracture or scar tissue around the implant causing a very unnatural appearance and hardness to the breast requiring surgical removal and implant replacement.

How is the Breast Implant Exchange surgery performed?

After your arrival to Serrão Surgical Center, you will be admitted and taken to the surgical suite, once you are fully prepped for the procedure as explained to you in your pre-operative visit.  Dr. Serrão will begin the Breast Implant Exchange Surgery after all the details and precise measurements that may be required are completed.  If possible, Dr. Serrão will use your incisions from the previous Breast Augmentation Surgery so you will not have another scar.  If the implants are being repositioned under the muscle and were above the muscle before, then Dr. Serrão will need to open a portion of the pectoralis muscle to create a pocket for the new implant to be placed.  If the implants are being repositioned over the muscle, Dr. Serrão will create a pocket over the muscle and under the breast tissue for placement of the new implant.

If you and Dr. Serrão decided to place the new implants into the existing pockets, there will be no need for the surgical development of a new pocket. If the existing pocket has become too large over the years, Dr. Serrão may be able to tighten the pocket so the new implant will shift less.  If you had developed a capsular contracture, Dr. Serrão will surgically remove the scar tissue.

When Dr. Serrão has properly placed the new implants, you will be raised into a sitting position so he can view the new implants and ensure your satisfaction with the surgical outcome.  Any adjustments will be performed if needed and the incisions will be closed and covered with surgical tape.

What type of anesthesia is used for Breast Implant Exchange Surgery?

Breast Implant Exchange Surgery is performed under deep sedation. Our certified registered nurse anesthetist (CRNA) can put you into a conscious sedation (gentle sleep) which will allow you to breath on your own, without crossing over into an unconscious state requiring a breathing tube and having greater risks. You feel nothing during the procedure and you will be safely monitored the entire time. We do these procedures in our fully accredited surgical center and your safety is always our priority.

What type of implants can be used for Breast Implant Exchange Surgery?

There are basically two types of implants that can be used for breast implant exchange, saline and silicone. You may choose to keep the type of implant that you had or replace them with a different type and shape.  Each has its own risks and benefits and that will be discussed during the consultation. In addition, natural breast augmentation with fat transfer can be performed shortly after the implants have been removed and you have healed. All these options should be discussed and considered before undergoing the procedure.

What happens post-operatively after Breast Implant Exchange Surgery?

Since tumescent anesthesia is used for the procedure, you will enjoy the benefit of continued numbness of the breasts for up to 6 to 8 hours after the procedure. The amount of pain you will experience is extremely variable, dependent on each individual woman. We have many options for post-operative pain relief and these will be discussed during your pre-operative visit.  Most women express that they had less pain with the secondary surgery than their primary breast augmentation surgery.

You will be discharged to home wearing a comfortable surgical compression bra. You can have some bruising, mild swelling and sensitivity to the breast as you had experienced in the primary surgery. You will be seen regularly so we can make sure all is going well. Dr. Serrão makes sure he is available for all his patients if necessary. What is the downtime with Breast Implant Exchange Surgery?

Most women return to office work in 5 to 7 days after cosmetic breast surgery.  You should avoid any strenuous activities that increase your heart rate or requires heavy lifting for a full six weeks.  A complete list of post-operative instructions will be given to you in your pre-operative instruction booklet.
